Goal: Improve public health care program value 
     Minnesota Health Care Programs’ cost increases
     Inpatient hospital average monthly cost per recipient
     Pharmacy average monthly cost per recipient
     Special transportation average monthly cost per recipient
Goal: Older Minnesotans will receive the long-term care services they need in their homes and communities, will be able to choose how they receive services, and will have more options for using their personal resources to pay for long-term care
      Proportion of nursing homes days paid by funding source
      Proportion of elders receiving publicly funded services in institutional versus community settings
      Proportion of public long-term care dollars expended in institutional versus community settings
   
Goal: Minnesotans with mental illness will receive timely and needed services in or near their own communities and will rely less on services in institutions
      Percentage of county-administered clients with a serious and persistent mental illness served in an institutional setting
      Average annual bed-days per client in an institutional setting
      Percentage of patients discharged from an institutional setting for serious and persistent mental illness readmitted to either an institutional or another inpatient setting within 30 days of discharge
 
Goal: More children will live in safe and permanent homes
      Adoption: Percentage of children who were adopted in fewer than 24 months from the time of the latest removal from their home
      Recurrence: Percentage of children who do not experience repeated neglect or abuse within 12 months of a prior report
      Re-entry: Percentage of children who entered foster care who did not have a prior out-of-home placement in the previous 12 months
      Reunification: Percentage of children reunified in less than 12 months from the time of the latest removal from their home
   
Goal: Reduce disparities in service access and outcomes
      For children enrolled in Minnesota public health care programs, percentage who receive the expected number of well-child visits
      Percentage of adults, by race and ethnicity, working at least 30 hours per week or off the cash portion of the Minnesota Family Investment Program (MFIP) three years after a baseline reporting period
